Funding Preference #1 and #2 Quality Home Care Services, Inc (QHCS) D/b/a Quality Comprehensive Health Center (QCHC) Is Proposing to Target and Recruit Existing and Future Community Health Workers (CHWS) and Health Support Workers Who Are From Disadvantaged Backgrounds (including Underrepresented Racial and Ethnic Minorities) to Join the Public Health Workforce in the Selected Community. Upon Completion of the Program, QCHC Will Work With Graduates to Secure Employment in Mecklenburg County, North Carolina, a Traditionally Underserved Area With a High Shortage of Health Professionals. the Purpose of the Proposed Program Is to Increase the Number of CHWS and Health Support Workers and Equip Them With the Skill Sets Needed to Provide Effective Community Outreach, Build Trust With Communities, Support Connections to and Retention in Care and Support Services, and Other Strategies to Increase Access to Care and to Assist Individuals in Prevention Services, and Recovery From the COVID-19 Pandemic and Other Public Health Emergencies in Underserved Communities. These Combined Efforts Are Intended to Advance Public Health, Strengthen the Public Health Workforce, Reduce Health Disparities, and Help Underserved Populations Achieve Health Equity. QCHC Will Target a Trainee Population That Is Reflective of Our Agency’s Target Patient Population and Those With the Greatest Need for CHWS - Historically Marginalized Low-income Black/african Americans, With a Focus on Individuals Who Are Representative of Some of the Most Difficult to Reach Populations, Including But Not Limited to MSM, Lgbtq+, Justice Involved, Hiv Positive/high-risk Negati Ve, Faith-based Community Members, Peers (including Individuals Recovered From Sud/oud and Those Who Have Experience Homelessness). QCHC Is Requesting a Funding Preference Under Preference 1 and 2 as Outlined in the Nofo. Total Funding Request for the Three-year Grant Period Is $1,238,783.
