Quality Improvement Fund - Justice Involved - Start Corporation (start), in Partnership With the Lafourche Parish Sheriff’s Office (lpso), Proposes This 2-YEAR Transitions in Lafourche Care (TLC) Program to Increase Access to Community-based, High-quality Primary Care Services for 400 Citizens Ages 18 and Older Who Have Been Incarcerated at the Lafourche Parish Correctional Complex (LPCC) and Are Reentering Southeastern Louisiana Regions 1, 2, 3, and 9. Within This 22-PARISH Service Area, Start Has Six Federally Qualified Health Centers (fqhcs), Five Assertive Community Treatment (ACT) Offices, Three Supportive Services for Veteran Families (SSVF) Offices, Two Ryan White Hiv Clinics, and One Certified Community Behavioral Health Clinic (ccbhc), Along With Its Vast Network of Housing Programs Including Permanent Supportive Housing (PSH), Rapid Rehousing (RR), and Rental Assistance. TLC Will Provide Individualized Case Management, Health Screening, and Care Coordination to Men and Women Within 90 Days of Their Reentry Date, Strengthening Their Transition Into the Community While Addressing Their Unique Health and Health-related Social Needs. Start Has a Forty-year History of Addressing Inequities and Providing Greater Access to Services and Opportunities for Communities That Have Been Historically Marginalized. as Such, Start Is Uniquely Positioned to Efficiently and Effectively Provide Post-release Medical and Behavioral Health Care Services, Individualized Case Management, and Wraparound Supportive Services Provided Primarily Through Its Internal Programs Including Securing Identification, Benefits Enrollment, Food, Housing, Education and Job Training, Transportation, and Legal Services. Combined, These Services Will Promote a Positive Reentry Experience and Reduce Recidivism Among Participants. This Program Will Be Independently Evaluated by Applied Research Services, a National Consulting Firm With Over 20 Years of Experience in Analyzing Correctional Agency Data for the Purposes of Designing Risk Assessment Instruments, Designing and Evaluating Graduated Sanction Programs for Probation Violators, Planning Future Staffing and Capital Outlay, and Evaluating Reentry and Other Justice System Intervention Programs.
