Purpose of the Award: This Grant Provides $97410 in Crrsaa FFY2021 5310 Funding for the Hattie Larlham Center and ST. Vincent Family Service. Matching Funds Were Revoked for This Funding Application by the Fta Due to Covid Emergencies. These Funds Will Be Used at a 100% Federal Share.activities to Be Performed: Projects Include Capital Vehicle Purchase Replacement and Purchased Transportation Activities Under the 5310 Program.expected Outcomes: Hattie Larlham Center With the Purchase of a New Replacement Van Will Enhance Their Transportation Services Offered to Their Clients for Work Medical and Enrichment Programs. ST. Vincent Family Services Through Purchased Transportation Funding Will Guarantee That Young Clients With Serious Mental Health Diagnoses Can Attend Critical Day Treatment Behavioral Services and Learn the Skills They Need to Move Forward Successfully in School and in Life.intended Beneficiaries: the Hattie Larlham Center Provides Services to Children and Adults With Intellectual and Developmental Disabilities. ST. Vincent Family Services Provides Services to Families and Individuals With Intellectual and Developmental Disabilities. Also Attached Is the 5310 Program Management Plan (PMP) and the Program of Projects (pop).in Executing This Grant the Mid-ohio Regional Planning Commission (MORPC) Agrees to Abide by Section 3011 of the Fast Act (49 U.s.c. 5323(J)(2)(C) Regarding the Phased Increase in Domestic Content Under the Buy America Statute for Rolling Stock.as Grant Agreement Includes an Fta-funded Transit Vehicle Purchase Recipient Agrees to Comply With the Related Requirements Set Forth in 49 CFR 26.49(A). Specifically Recipient Must Ensure as a Condition of Being Authorized to Bid or Propose on Fta-assisted Transit Vehicle Procurements That Each Transit Vehicle Manufacturer Certified That It Complied With Dot Requirements for Submitting a Dbe Program and Goal to Fta Prior to Bidding. Recipient Is Also Required to Submit the Name of the Successful Bidder and the Total Dollar Value of the Contract Within 30 Days of Making an Fta- Assisted Transit Vehicle Award Using the Required Fta Reporting Tool. Recipient Understands That Failure to Comply With These Requirements May Result in Formal Enforcement Action or Appropriate Sanction as Determined by Fta Including Reimbursement of Fta Funds Used for the Vehicle Procurement.
