Comprehensive Services for Reducing Hiv Infections Among Blacks/african Americans in Palm Beach County, Florida - Foundcare, Inc. Is Proposing to Reduce the Number of New Hiv Infections Among Blacks/african Americans in Palm Beach County, Florida by Implementing a High Impact Hiv Prevention Program That Includes Targeted Hiv Testing, Linkage to Care for Those Who Are Newly Diagnosed or Living With Hiv and Out of Care, Providing Access to Other Prevention and Support Services, and Condom Distribution. Foundcare Proposes to Test 3,240 Blacks/african Americans Over the Five-year Grant Period and Link at Least 36 Newly Diagnosed or Living With Hiv and Out of Care Into Hiv Medical Services. Foundcare Implements a Test and Treat Approach to Link People Into Hiv Medical Care Within 72 Hours of Receiving and Hiv Positive Test Result, Whenever Possible. These Activities Will Take Place Throughout Palm Beach County in Targeted Areas as Determined by a Community Engagement Committee Consisting of Members of the Target Population Who Advise Foundcare on Its Hiv Prevention Services. Foundcare Will Collect and Report Data on Each Activity and Monitor Program Progress on a Monthly Basis.
