African Americans Are the Most Affected Racial/ethnic Population in the United States, the South and Consequently the New Orleans Eligible Metro Area (no Ema). PHC Proposes to Provide High-impact Hiv Prevention Services to High-risk Heterosexual (HRH) African American Males and Females, Ages From 13 to 49, Living in the No Ema, Placing a Special Emphasis on Jefferson Parish. Short Term Goals: 1) Prevent New Infections, 2) Improve Health Outcomes for PLWH and 3) Prevent New Infections. at Least 600 People Are Served in Year 1 and 3,000 People by End of Year 5. Short Term Outcome Measures Include Year 1 and Year 5 Outlined in the Project Narrative:?outcome Measure: a Minimum of 75% of Persons Tested Must Be Part of the Recipient?s Selected Target Population?outcome Measure: at Least 1.6% of Persons With Newly Diagnosed Hiv Identified Through PS21-2102 Funded Testing?outcome Measure: at Least 75% of PS21-2102 Hiv Testing Events Conducted in Which the Client Was Provided Screening for STDS (syphilis, Gonorrhea, Chlamydia), Viral Hepatitis, or TB (in Conjunction With PS21-2102 Funded Hiv Testing).?outcome Measure: 90% of Persons With Newly Diagnosed Hiv Will Be Linked to Hiv Medical Care Within 30 Days of Diagnosis.?outcome Measure: at Least 27% of Persons With Previously Diagnosed Hiv, Not-in-care, Linked to or Re-engaged in Hiv Medical Care Within 30 Days.?outcome Measure: 100% of Persons With Newly Diagnosed Hiv Will Be Referred to Partner Services, in Accordance With State and Local Regulations.?outcome Measure: at Least 78% of Persons Re-engaged in Care Will Participate in Our Rapid Restart Program.
