Comprehensive High-impact Hiv Prevention Program for Community-based Organizations - Founded in 2005, Quality Home Care Services (QHCS) Is a Non-profit 501(C)3 Organization Serving a Multi-county Region That Touches Seven Designated Medically Underserved Areas (MUAS) in and Surrounding Charlotte, North Carolina. QHCS Is Dedicated to Addressing Various Health Related Challenges That Significantly Affect the Community?s Health and Wellness. in Conjunction With Its Health and Human Service Initiatives, the Organization Offers a Full Continuum of Care Ranging From Initial Outreach to Service Provision. QHCS? Continuum of Services Includes a Full Hiv Continuum of Care/treatment Cascade, Primary Health Care, Hiv/sti Testing, Prevention, Medical Case Management and Counseling, Substance Use/misuse Treatment, Mental Health Services, and In-home Care. QHCS Is Committed to Providing Evidence-based Interventions and Culturally Competent Comprehensive Health Care to Underserved Patients Throughout the Greater Charlotte Region. QHCS Is Applying to the Centers for Disease Control for Funding Under CDC-RFA-PS21-2102: Comprehensive High-impact Hiv Prevention Programs for Community-based Organizations. QHCS? Proposed Program Will Support Progress Along the Hiv Care Continuum to Improve the Health, Adherence to Care, and Life Span of Hiv Positive and High-risk Black/african American MSM. Our Proposed Program Will Accomplish the Outcomes and Goals for PS21-2102, Including: - Diagnose: Provide Targeted Hiv Testing and Integrated Screening.- Treat: Provide Linkage to Hiv Medical Care, Re-engagement, Partner Services, and Medication Adherence (to Achieve Viral Suppression).- Prevent: Provide Referrals to Prep/npep, Health Education/risk Reduction, and Condom Distribution.- Respond: Provide Support for the Health Department Cluster Detection and Response Activities. in Addition, Program Promotion, Outreach, and Recruitment, CDC Recommended Evidence-based Interventions and Referral to Essential Support Services Will Accompany All Project Activities.q HCS? QHCS Supports Ending the Hiv Epidemic: a Plan for America (EHE) by Leveraging Scientific and Evidence-based Advances in Hiv Prevention, Diagnosis, Treatment, and Outbreak Response. for the Past Ten Years, QHCS Has Been Instrumental in Contributing Surveillance Data and Prevention Efforts in the Geographic Hotspot, Mecklenburg County. Evidence of Our Contributions Can Be Seen by Our Coordinated HHS Programming, Where We Leverage Grant Programs From Hrsa, CDC, Samhsa as Well as State and County Efforts to Diagnose, Treat, Prevent and Respond.
