Treatmentlink - Center Point, Inc. (CPI) Is Requesting $500,000 Per Year for Up to Five Years From the Samhsa Center for Substance Abuse Treatment to Expand Engagement and Increase the Availability of Comprehensive Residential and Outpatient Treatment; Prevention; Care; Recovery Support Services; and Harm Reduction Interventions for Racial and Ethnic Underrepresented Individuals With Sud and Cod Who Are At-risk for, or Who Are Living With Hiv/aids. the Population of Focus for the Treatment Link Project Is Low-income African American and Hispanic/latino Men and Women Who Are Homeless And/or Who Have Released From Incarceration Within the Past Year and Who Have Limited Access to Quality Primary and Specialty Care or a Comprehensive Treatment Service System for Sud and Cod. the Geographic Catchment Area Where Project Services Will Be Delivered Is the San Francisco, Alameda, and Marin County and the Greater Bay Area, California. Treatment Link Will Expand Access and Admit an Additional 15 Participants Per Year; a Total of 75 Unduplicated Participants Will Be Admitted to Treatment Link. the Goals of Treatment Link Are to Conduct Outreach Strategies in the Identified Communities to Identify, Engage, and Admit Members of the Focus Population Into Residential and Outpatient Treatment; Deliver an Effective, Evidence-based, Comprehensive System-of-care, Including Trauma-informed Service, Harm Reduction, and Recovery Support Services; Improve Treatment and Recovery Outcomes and Quality of Life for Those With or At-risk of Hiv/aids and Their Partners; Provide Primary and Specialty Medical, Mental Health, Vocational, and Educational Programs; and Increase Cpi's Sustainable Capacity and Expand the Availability of a Comprehensive Continuum of Care That Promotes Recovery and Independence for Participants. Cpi Will Conduct Comprehensive Sud, Cod, and Physical Health Screenings and Assessments at Intake and Continuously to Aid in the Diagnosis of Medical Necessities, Treatment Planning and Clinical Needs, Medications, Including Medication Assisted Treatment (mat). Cpi Will Embed Treatment Link Components Within a Comprehensive Multi-modality System-of-care, Including Carf Accredited, DHCS Licensed and Certified Asam Levels of Care for Outpatient (1.0, 2.1) and Residential (3.1, 3.2WM, 3.2, 3.5), and Recovery Residences. Treatment Link Will Enhance the Quality and Intensity of Integrated Primary and Specialty Care and Mental Health Treatment Provided Through Expanded Care Coordination and Case Management at Cpi's Co-located Clinic Partnership With the Marin Community Clinics, FQHC. Cpi, With 50+ Years of Experience in the SF Bay Area, Will Serve as the Primary Provider of All Sud and Cod Treatment Services and Will Provide Coordinated Care Coordination and Collateral Support to Those Referred for Confirmatory Hiv Testing And/or Treatment.
