Mai Substance Use Disorder Treatment for Racial/ethnic Minority Populations at High Risk for Hiv/aids in Liberty City, Miami, FL - Care 4 U Management, Inc. D/b/a Care 4 U Community Health Center Proposes to Increase Hiv Testing and Engagement in Care for Black and Hispanic Individuals With Substance Use Disorders (SUD) And/or Co-occurring Disorders (COD) Who Are at Risk for, or Are Livign With Hiv/aids Nad Receive Services/treatment in the Liberty City Area of Miami-dade County, FL. We Will Accomplish This by Providing: (1) Testing for Hiv, STDS and Viral Hepatitis, (2) Primary Medical Care for Those Who Test Positive for Hiv, STDS, And/or Viral Hepatitis, (3) Prep to Those Who Test Hiv Negative and Are at High Risk for Hiv Infection, (4) Case Management Linkage and Referral Services for Individuals With Sud/cod, Regardless of Hiv Status, and (5) Screening for Sud/cod by Medical Providers And/or Hiv/hcv Testing Counselors. We Will Partner With Licensed Substance Abuse Provide, Community Rightful Center (CRC), Another Minority-based Nonprofit Organization. CRC Will Provide Treatment for Substance Abuse and Mental Health Counseling at Care 4 U's Medical Clinic in Liberty City. Miami-dade County, FL Is Onle of the Localities Hardest Hit by the Hiv Epidemic. Care 4 U's Cachment Area, Liberty City, Is the Epicenter of the Hiv Disease in the County. Care 4 U's Efforts Will Target Black and Hispanic Adults in Liberty City and Surrounding Zip Codes, Which Are the Most Economically and Socially Depressed Neighhoods in Miami-dade County. the Services Propsoed to Care 4 U Seek to Ameiiorate the Affects of Hiv and Substance Abuse Among Black and Hispanic Individuals in Lieverty by Increasing Access to Testing, Treatment and Prevention With the Goal of Improving Health Outcomes and Reducing Hiv Transmission Rates Among This Popualtion. We Propose to Screen 960 Individuals in Year 1, Increasing That Number by 5% Each Subsequent Year, for a Total of 5,300 Individuals Over the Five Year Grant Period. We Are Requesting $474,408 a Year for a Total of $2,357,640 Over the Five Year Grant Period.
