Fy 2020 Grants for the Benefit of Homeless Individuals - the Purpose of This Program Is to Support the Expansion and Implementation of the Middlesex County Mental Health and Addiction Community Infrastructure That Integrates Substance Use Disorder Treatment, Housing Services and Other Critical Services for Individuals (including Youth) and Families Experiencing Homelessness. the Proposed Services Will Meet the Goals and Objectives Through the Direct Provision of Community Outreach and Clinical Outpatient Services Provided in Coordination With County Housing, Mental Health and Addictions Service Providers. Our Goals for the Proposed Project That Are Aligned With the Statement of Need Are: Goal 1: Increase Access to Housing Among Individuals Experiencing Marginal Housing or Homelessness. Objective 1A: 100% of Homeless Individuals Served Will Receive Direct Support and Assistance to Secure a Stable Living Environment. Objective 1B: 90% of Homeless Individuals Served Will Be Safely Sheltered. Goal 2: Increase Engagement in Evidence-based Direct MH and Sud Screening and Treatment for Objective 2A: 100% of Homeless Individuals Served Will Be Screen for Suds and MH Needs. Objective 2B: 100% of Homeless Individuals Screened Positive for Suds/mh Will Be Connected to Treatment at the Outpatient, Iop, PH, or Inpatient/residential Level of Care. Objective 2C: 80% of Homeless Individuals Screened Positive for Suds/mh Will Enter Treatment. Goal 3: Increase Access to Permanent Housing Objective 3A: 100% of Homeless Individuals Served Will Be Provided With Linkage and Coordination to Obtain Permanent Housing. Objective 3B: 25% of Homeless Individuals Served Will Enter Permanent Housing. Goal 4: Increase Enrollment in Benefit Programs Among Individuals Experiencing Marginal Housing or Homelessness. Objective 4A: 100% of Benefit Eligible Individuals Will Be Supported to Enroll in Medicaid And/or Other Benefit Programs (e.g. Ssi/ssd, Ga/tanf, Snap). Objective 4B: 40% of Individuals Will Be Enrolled in Medicaid And/or Other Benefit Programs. Goal 5: Increase Engagement in Recovery Support Services Among Individuals Experiencing Marginal Housing or Homelessness. Objective 5A: 100% of Individuals Served Will Be Offered Recovery Coaching. Objective 5B: 80% of Individuals Served Will Increase Reported Sense of Social Connectedness. Objective 5C: 40% of Individuals Served Will Receive Employment Coaching. Objective 5D: 30% of Individuals Served Will Be Referred to Recovery Housing the Program Will Serve 100 Unduplicated Individuals Annually, and 500 Unduplicated Individuals Over the Entire Project Period.
