The Nashville SPF RX Program - Street Works of Nashville, Tennessee, Will Implement the Strategic Partnership Framework (SPF) Culturally Informed Approach to Expand and Strengthen the Community's Capacity to Address RX Substance Misuse. the SPF RX Grant Program Will Use a Multiple Level Approach to Prevent and Mitigate Prescription Misuse by Implementing Education and Assessments at the Individual, Organization and Community Levels. We Will Tailor Our Approach by Carefully Curating Educational Programming for the Focused Populations. These Populations Include Community Residents County Wide and in North and South Nashville, Community Based Organizations and Medical and Dental Providers. the Partnership Will Be Built Within the Nashville Thrives Coalition Forming a Particular Interest Group Focusing on RX Misuse. We Will Engage a Targeted Outreach to the North and South Communities Through the Street Works Outreach Program and Metro Public Health Department Community Health Worker Program. We Will Assess, Educate and Provide Navigation to Services as Our Secondary Prevention Approach. at the Organization Level, We Will Build the Coalition by Inviting Medical and Dental Providers, Housing Developments, Faith Communities and Others to Participate by Implementing Assessments and Education. We Will Also Engage an Educational Community for Youth and the Larger Community Regarding RX Safety, RX Sharing, Online and Street Purchases and Other Health Related Risks. as a Primary Focus, We Will Educate Organizations and Providers on the Controlled Substance Monitoring Data (CSMD) Program to Increase Awareness and Use. We Will Engage the CSMD Program to Share What Is Learned Through Our Community Data Collection Efforts That Can Inform the Current Information Collected and Reported. a Continuous Quality Improvement Approach Will Be Applied to All Activities Within the Program to Facilitate the Effectiveness of Efforts. by Strengthening the Network of Providers Who Learn About the CSMD and the Results of Assessments Across the Communities and Their Patients, We Can Improve and Reduce RX Drug Misuse and Progression to Other Substances and Increase Safety for Families. by Focusing on Our Black, African American, Latinx and Immigrant Communities, We Have the Opportunity to Empower the Populations to Prevent and Mitigate the Adverse Effects of RX Drug Misuse.
