Ccbhc-evidence Based Outpatient Therapy Services - Samsha Fy 2022 Certified Community Behavioral Health Clinic (CCBHC) Improvement and Advancement Grant Genesee Health System Ccbhc Abstract Summary the Genesee Health System (GHS) Ccbhc Will Serve Residents of Genesee County, Michigan Experiencing Mild to Moderate Behavioral Health Care Needs Through Development of an Outpatient Clinic. the Intended Goal of This Project Is to Offer Timely Therapy to Individuals With Mild/moderate Mental Illness. Currently, There Is Not Timely Access to Therapy for Individuals in Our Community, Especially Uninsured and Underinsured Persons. GHS Aims to Provide Same-day Intake Assessment or Immediately Schedule an Appointment for Those That Present With Routine Needs. the Individual Will Then Be Enrolled in Outpatient Therapy Using Care Pathways and Evidence Based Practices (EBPS) Based on Diagnoses and Presenting Problem. This Project Aims to Provide Vulnerable Residents With Increased Access to Evidence-based Practices. to Support the Provision of Quality Care, Clinical Staff Will Receive Additional Training in Evidence-based Models of Care. Specifically, Staff Will Be Trained in Ebps for Common Diagnoses to Ensure the Clinic’s Ability to Treat Everyone Who Requests Services. Training Will Include Models of Therapy Such as Cognitive Behavioral Therapy (CBT), Dialectal Behavioral Therapy (DBT) Informed Treatment, and Motivational Interviewing. This Allows Anyone That Presents on the Spectrum of Need to Have Access to a Full Array of Clinically Appropriate Services. Individuals in These Communities Continue to Face Socioeconomic Hardship, Environmental Challenges, Complex Trauma as Well as a Decreased Life Expectancy Due to Social Determinants of Health Needs. It Is GHS’S Expectation That Engaging These Individuals in a Continuum of Care That Seeks to Prevent the Escalation of Mental Health Symptoms Before It Worsens, Will Minimize Psychiatric Recidivism, Increase Jail Diversion, and Improve Individuals’ Overall Quality of Life. This Project Will Be Evaluated Through the Collection of Data, the Use of Standardized Tools and Assessments, and Pre-post Intervention Screenings.
