Pecan Valley Centers CMHC Grant Project - Pecan Valley Centers' CMHC Grant Program Will Significantly Improve the Lives of the Individuals We Serve by Expanding Telehealth Capabilities, Increasing Outpatient Services for Individuals With Sed, Smi, and Cod, Increasing Clinical and Recovery Support Services, as Well as an Expanding the Employee Wellness Program. Pecan Valley Centers Will Focus on the Uninsured, Low-income Children and Adults Living in Our Service Area That Have a Diagnosis of Sed, Smi, and Cod or Substance Use Disorders Who Have Co-morbid Physical Health Conditions and Chronic Diseases. Pecan Valley Centers Will Increase Access to Mental Health Prescribers, Implement a Primary Care Program, Increase Access to Benefits Specialists, Address Transportation Barriers, Increase Intake Process for Individuals in Crisis, and Expand Outpatient Substance Use Disorder Treatment Program. Additionally, Pecan Valley Centers Will Increase Staff and Community Knowledge of Suicide Prevention and Expand the Employee Wellness Program. by Increasing Access to Mental Health Prescribers, Pecan Valley Centers Proposes to Serve 400 Unique Individuals Each Grant Year, for a Total of 800 Over the Lifetime of the Grant. by Implementing the Primary Care Program, Pecan Valley Centers Proposes to Serve 200 Unique Individuals Each Grant Term, for a Total of 400 Over the Lifetime of the Grant. by Increasing Access to Benefit Specialists, Pecan Valley Centers Proposes to Serve 150 Unique Individuals Each Grant Year, for a Total of 300 Over the Lifetime of the Grant. by Addressing the Transportation Barriers, Pecan Valley Centers Proposes to Provide Transportation Services to 330 Unique Individuals Each Grant Year, for a Total of 660 Over the Lifetime of the Grant. by Increasing Intake Access to Individuals in Crisis, Pecan Valley Centers Proposes to Serve 350 Unique Individuals Each Grant Term, for a Total of 700 Over the Lifetime of the Grant. by Adding a Recovery Peer Support Specialist to the Outpatient Substance Use Disorder Treatment Program, Pecan Valley Centers Proposes to Serve 70 Unique Individuals Each Grant Year, for a Total of 140 Over the Lifetime of the Grant. a Total of 3000 Individuals With Sed, Smi, and Cod Will Be Served Over the Life of the Grant.
