The Cincinnati Health Care Integration Program (CHCIP) - Established in 2008, Exclusive Services (ES) Is a Non-profit, Community and Integrated Healthcare Agency, With Over 12 Years of Extensive Experience Providing Prevention, Intervention and Treatment Services to Individuals and Families Affected by Substance Abuse and Mental Health Disorders; Hiv/aids; Obesity and Other Supportive Services for People From City of Cincinnati, Hamilton County, Ohio. This Grant Application Requests Funding Support in the Amount of $2,000,000.00 Per Year for Two Years to Develop/expand/establish the Cincinnati Health Care Integration Center (CHCIP) Designed to Improve the Health of the City of Cincinnati’s and the State of Ohio and the Underserved Communities and Vulnerable Populations. to This End, the Proposed Community Health Center Plans to Provide Comprehensive Services to Address the Major Health Care Needs of the Target Population. These Health Care Needs Include But Are Not Limited to Lack of Access to Prenatal Care in the First Trimester; Lack of Access to Appropriate Immunizations; High Rates of Obesity, Diabetes, Hypertension; High Rates of Depression; High Rates of Substance Abuse; and Homelessness. Our Vision Is That Ensuring That the Target Populations Are Healthy and Reaching Their Full Potential Requires the Prevention of These Diseases; the Promotion of Health and Well-being; the Assurance of Condition in Which People Can Be Healthy; and the Provision of Timely, Effective, and Coordinated Health Care. to This End, the Project Will Therefore Aim at (1) Ensuring Access to Comprehensive, Culturally Competent, Quality Primary Health Care Services to the Medically Underserved Target Populations; (2) Ensuring the Availability of Essential Primary Care and Preventive Health Services (including Oral Health, Mental Health, and Substance Abuse Services as Well as a Variety of Supportive and Enabling Services Such as Translation/interpretation, Case Management, Street Community Outreach, Nutrition, and Transportation Services; (3) Establishing a Sustainable Collaborative and Coordinated Primary Health Care Delivery System for the Provision of Health Care to the Targeted Underserved Population and (4) Improving Patient Health Outcomes. Currently, Es Provides Services to Over 1,600 Individuals Annually Comprising a Total of Over 8,000 Visits for an Average of Five Visits Per Client Per Year. Presently, Es Provides Integrated Health Care Services at One Site Where the Health Care Center Will Be Established in an Effort to Improve Quality of Care, Accessibility, Affordability and Availability of Health Professionals. the Proposed Site Will Serve at a Minimum 500 Patients Annually. Services to Be Provided Include But Will Not Be Limited to Primary Health Care, Mental Health/psychiatric, Substance Abuse, Hiv/hep Testing, Dental, Enabling Services and Pharmaceutical Services.
