Healthy Start Initiative - Black Birth Justice Is Partnering With Healthy Beginnings Connect Us to Provide the Healthy Start Program to Black and Latinx Families That Experience the Highest Disparities in Perinatal Outcomes Across Louisville Metro. We Are Excited to Transform the Current Healthy Start Programming Delivery Model in Louisville, Ky, Into a Culturally and Generationally Relevant, Client-driven, Efficient, Flexible, Program Rooted in Equity, Lived Experience, and Community Accountability. the Healthy Start Program Increases Access to Direct and Enabling Services for Pregnant People and Families With Infants in the Target Population and Coordinates the Preventative Care and Early Intervention Partners in Our Community to Eliminate Disparities in Perinatal Health, Improve Birth Outcomes, Reduce Infant Mortality, and Promote Family Health and Well-being. as Former Healthy Start Participants With Lived Experiences, We Incorporate the Following Values Into Delivering Healthy Start Direct and Enabling Services, Case Management, and Coordination: - Promoting Client Empowerment and Decision-making - Results-driven Service Planning - Effective Coordination & Collaboration - Nurturing Natural Support Systems - Ensuring Seamless Continuity of Care Services Provided Include: - Ensuring Participants Have Access to Preventive Health Services, Behavioral Health Care, and Other Specialty Services - Case Management/care Coordination Services - Group-based Prenatal/postpartum and Parenting Education - Health Promotion - Continuum of Services Across Perinatal Stages We Request $5,500,000 in Funding Over 5 Years in This Request
