Diabetes Program for Yavapai-apache Nation - Diabetes Treatment and Prevention Is Provided Through the Yavapai-apache Nation Medical Clinic for All Yan Members and Other Ai/an in the Local Area, Also From the Yan Sdpi Program Through Education and Activities. the Yavapai-apache Nation Is Comprised as 5 Scattered Sites in Rural Areas - Middle Verde Community, Camp Verde Community, Tunlii Community, Which Are All Located in Camp Verde Az, Rimrock Community, Located in Rimrock Az, Clarkdale Community, Located in Clarkdale Az. the Yan Community Members Receive Diabetes/medical Services at Our Local Yan Medical Clinic, Other Services Provided Are From Phoenix Indian Medical Center. for Specialty Services Either Provided at Pimc or Referrals From Yan Medical Clinic to Outside Facilities Upon Approval. as for Activities the Diabetes Program in Collaboration With the Wellness Program Provide Activities for Community Members in the Two Larger Communities in Middle Verde and Clarkdale.
