Eastern Shoshone Tribal Health Special Diabetes Program - Our Sdpi Team Continues to Provide Quality Work in Order to Fulfill the Changes of COVID-19. Eastern Shoshone Tribal Health's Special Diabetes Program Is a Organization Working Towards Obtaining a Spot as a Nationally Recognized Diabetes Prevention Program With the Centers for Disease Control (CDC) and the Diabetes Prevention Recognition Program (DPRP). Our Status Is Currently in the Preliminary Status. We Have a Completed Two Successful Cohorts With 30 Participants Enrolled in the Prevent T2 Curriculum. Our Third Cohort Starts on November 15, 2022. We Have Now Moved Into the Final 1 of 4 Reporting Sequence to Obtain Our Full Status, This Is a Two Year Process. the Program Gives Our Participants a Full Year of Support to Make Lasting Lifestyles Changes That Improve Their Overall Health and Reduce the Risk of Type 2 Diabetes and Is the CDC'S Approved Curriculum. Our Staff Are Trained Lifestyle Coaches and Personal Trainers Who Support Each Participant on an Individual Level and as a Group. the Participants Have Even Been Able to Manage Other Health Conditions Like High Blood Pressure and High Cholesterol With Fewer Medications. the Sdpi Staff Track and Manage Data Collections of All Activities Implemented Under the Eastern Shoshone Tribe Health Programs. the Data Is Then Entered Into RPMS and Filed in the Ehs System and Patient Record.
