Community Project Funding/congressionally Directed Spending - Non-construction - 5700 Thurston Ave Suite 101, Virginia Beach, Va 23455 Project Director: Christian Joyner Phone Number: 757-675-2719 Cjoyner@stopinc.org Www.stopinc.org Stop Inc.’s Mobile Telehealth Clinic (MTC) Will Bring Medical Services to Uninsured and Under Insured, Low to Moderate Income Residents in the Rural Areas of Western Tidewater. These Services Include Medical, Vaccines, Dental, Mental Health and Substance and Opioid Use Disorder (sud/oud). It Will Provide Management of Acute and Chronic Medical Conditions Including, But Not Limited to, Acute Illness, Sexually Transmitted Disease (STD) Exposure, Diabetes, Hypertension, Chronic Obstructive Pulmonary Disease (copd)/asthma, Renal Failure, Gynecological, Congestive Heart Failure, and Coronary Artery Disease. Mental Health Services, to Include Medication Management and Individual Therapy Will Be Available Through the MTC’S Primary Care and Psychiatric Services. Dental Services Are Greatly Needed in the Area. It Is Anticipated the MTC Will Offer Basic and Emergency Dental Services to Our Clients in Future. It Has Been Our Observation That These Rural Areas Lack Access to Regular Medical and Dental Care and the Target Population Does Not Demonstrate Understanding of the Long Term Consequences of Their Uncontrolled Multiple Illnesses. the MTC Will Park at Sites Within Walking Distance of Targeted Clients. Standard Preventative Services That Address Preventative Screenings and Routine Vaccinations, for Example Mammogram, Annual Prostate Exams, and Colonoscopy, Will Be Recommended and Referred. Beyond Those Routine Screening Recommendations, Stop Intends to Take a Step Further to Educate and Empower the Clients to Use the Power of Lifestyle as Preventative Medicine During Primary Care Visits. Incorporating Nutrition Education as a Tactic for Preventing and Combating Disease Is Lacking in the Area. Stop Anticipates Hiring a Part Time Nutritionist on the MTC to Fill This Gap. Age Appropriate M Ental, Medical and Dental Pre-assessment Screenings Will Be Completed on Each Client Served to Determine a Baseline of Each Individual’s Overall Status of Health. by Utilizing the Concept of Precision Medicine, the Service Provider and Client Will Mutually Develop an Individualized Plan to Achieve Holistic Health Goals. MTC Case Managers Will Further Work With Patients on Individual Recommendations. Measurable Data Points to Demonstrate Improvements in Each Individual’s Health Will Be Collected, Evaluated, and Discussed With Each Client. Health Insurance Portability and Accountability Act (HIPAA) Compliant Data Will Be Collected to Identify Notable Outcomes in the Overall Welfare and Health of the Targeted Clients for Reporting and Needs Assessment. Sud/oud Screenings, Education, Medication for Opioid Use Disorder (MOUD) With Suboxone and Vivitrol Will Be Available. Rapid Human Immunodeficiency Virus (HIV) and Hepatitis C Screenings Will Be Offered on Demand and During Routine Screenings. COVID-19 Has Certainly Exacerbated Already Challenging Situations for This Very Vulnerable Population. Stop’s MTC Will Address the Additional Complications, Including the Mental Health Impact Surrounding Loss and Quarantine Due to COVID-19. the MTC Will Also Offer Regular Rapid Antigen Testing and COVID-19 Vaccinations. in Each Subset of Services Offered, Telehealth Capabilities Will Be Offered. It Is Stop’s Intention to Utilize the Unit’s Telehealth Resources in Order to Aid in Patient’s Disease Management With Specialists and Other Outside Referrals or During Any Future Needs for Quarantine. Stop's MTC Will Bring Much Needed Health Care to the Area and Aims to Improve the Long Term Outcomes for Its Underserved Target Population.
