Medstar Harbor Hospital Is Committed to Providing a Quality, Caring Experience for Our Patients, Our Communities, and Those Who Serve Them. Medstar’s Mobile Health Center Is a 40FT Freightliner That Debuted in Fall 2020. With Walk-ins and Appointments at Strategic Community Sites Across South Baltimore, the Center Provides Critical Health Services Outside of Hospital Walls to Historically Marginalized Communities. the Center Travels to Convenient Locations Including Cherry Hill, Brooklyn, Westport, and Curtis Bay/hawkins Point, Locations Chosen With Input and Guidance From the Communities on a Regular, Well-advertised Schedule. This Clinic on Wheels Has a Unique and Powerful Purpose: to Provide Greater Access to Care, and to Better Meet the Needs of the Communities We Serve. Care Services Include Primary Care, Preventative Care, Routine Screening, Chronic Disease Management, and COVID-19 Vaccine Distribution. the Center Operates Over 30 Hours Each Week (monday-thursday) Visiting 8 Locations in Baltimore City and Anne Arundel County. Decisions for Each Site Location Were Based on the Needs and Vulnerabilities of the Population Served and Leverages Community Partnership to Offer Patients Resources to Address Social Needs. in Addition to Clinical Care, the Center Employs a Community Health Advocate Who Connects Patients to Supplemental Support Beyond Standard Health Services Such as Housing, Behavioral Health, Food Access, Job Assistance, Substance Use Treatment, and Other Social Services. the Requested Funding Will Cover One Ye Ar of Annual Operating Costs of the Mobile Health Center and the Purchase of an Additional Mobile Health Center to Expand Primary Care Services to the Community, Addressing Chronic Disease Management, Prevention, and Vaccine Distribution. This Will Include Greater Focus Coordination and Linkages to Care Between Visits. Many of These Interactions Will Be Improved by the Ability to Hold Virtual Visits to See the Patient While Discussing Their Concerns Through Telehealth Care. Through This Initiative, the Aim Is to Eliminate Common Barriers to Both Real and Perceived Healthcare Access and Move Towards Improved Population Health and Health Equity for Some of Our Region's Most Vulnerable Residents.
