GOALS:1) Replicate, Test, and Refine the Evidenced-based Washington County Mihn Model for Behavioral Health and Preventable Hospital Readmissions in Franklin and Lincoln Counties Within the High Need/high Utilizer Population. 2) Increase Workforce Education, Training, and Capacity That Supports Mihn Service Delivery to Improve Population Health and Lower Overall Costs to the Health Care System. 3) Develop Strong Relationships and Partnerships Between Healthcare Providers and Key Local, Regional, and State Stakeholders to Further Support Replication, Expansion, and Sustainability of the Mihn Model in Missouri. Focus Area(s): Behavioral Health and Co-occurring Conditions; and Preventable Hospital Readmissions. the Service Area Is the Rural Communities of Franklin and Lincoln Counties in Eastern Missouri. the Target Population Is High Need/high Utilizers of Emergency Services at Risk of Preventable Hospital Readmission and Individuals With Behavioral Health and Other Co-occurring Conditions. Eastern Missouri Mobile Integrated Healthcare Network Is Comprised of Seven Members: Uad, Meramec Ambulance District, ST. Clair Ambulance District, Lincoln County Ambulance District, Mercy Hospital Washington, Mercy Hospital Lincoln, and Compass Health Network. the Network Represents Health Care (fqhcs), Hospitals, and Ems. the Activities Include Network Infrastructure Development That Allows for Data Sharing, Consensus Decision-making, Standardized Treatment Protocols and Process for Referrals; Provision of Chw/ibhs and Community Paramedic Services to Improve Access to Care and Resources to Address Social Determinants of Health. Activities Also Include Training and Education on the Model and Evidence-based Practices to Ensure Quality Healthcare and Health Equity. Expected Outcomes Include: I) Improved Access to and Quality of Health Care Delivery for High Need/high Utilizer Populations That Lead to Improved Health Outcomes. Ii) Expand Capacity and Services Through Increased Enrollment in Medicaid Program and Other Forms of Health Insurance; Increase Access to Resources That Address Social Determinants of Health. Iii) Enhance Outcomes Through Improved Patient Engagement, Self-management, and Medication Reconciliation; Reduced Spread Of/exposure to COVID-19 by Safeguarding High-risk Patients in Their Homes. Iv) Sustainability Through Replication of the Mihn Model for Behavioral and Co-occurring Conditions, and Preventable Hospital Readmissions, With Demonstrated Cost Savings That Can Be Replicated Statewide; Reduction in Ed Transports for High Utilizers of Ed Services. Uad Has Extensive Experience Working With Rural and Underserved Populations, Touching Lives in Every Possible Arena, and at the Worst of the Worst Times. Collaboratively, the Network Has the Expertise and Capacity to Implement the Scope of Work Proposed. Uad Will Bring Leadership and Financial Reporting and Accounting Services to Support the Network. the Network Is Requesting a Funding Preference of Hpsa Designation, With the Data From the Hrsa Hpsa Find Documented in Attachment 12 of the Application Submission.
