Focus Area Unaccompanied Youth, Homeless Youth, Health Outcomes Target Patient Population Our Network Will Be Primarily Operating in Albany County, Wyoming, Usa But Also Be Operational to Serve All the Surrounding Rural Areas in the State. Our Target Patient Population Is Unaccompanied Youth, Often Referred to as Homeless and All Subpopulations Within This Already Vulnerable and Hidden Population. Network Members Network Name: Unaccompanied Students Initiative’s Rural Health Network Total Number of Member Organizations: 3 With a Potential to Include 3-5 More Network Project Activities/services Our Activities Include the Following: Continue Promoting, Designing, and Implementing Interventions Grounded in the Social Determinants of Health in Alignment With National Health People 2030 Goals to Improve Health Outcomes Among Rural Youth; Lead the Development and Implementation of Project Echo (extension for Community Healthcare Outcomes), an All Teach All Learn Telecommunication Case-based Learning Program Uniting the Key Stakeholders Across Rural Areas for the Provision of Targeted Care to Homeless Youth and Unique Sub Populations and; Raise Awareness and Education Among the General Population About Homeless Youth and Increase Knowledge About the Organizations Who Are Able to Provide Them Assistance and the Services Available to the Youth. Expected Outcomes Our Expected Outcomes Include: Improved Health and Quality of Life Outcomes for Unaccompanied Youth and Individuals Encountering Them at Various Levels of Intervention; Improved Health of Unaccompanied Youth Through the Capacity Building of Medical and Nonmedical Service Providers, Increased Access to Specialized Providers, and Promotion of Resources Sharing in Rural Communities; and Offer Sustainable Community Developed Support Services and Offerings to the General Public and Global Community. Capacity to Serve Rural Underserved Populations the Unaccompanied Students Initiative (usi), a 501C3 Non-profit (ein: 83-2885380), Became the Focus of Strong Families Action Team Due to a Need Identified Through the Point in Time Count and Mckinney-vento Data Through Laramie County School District #1. Due to the Need for Homeless Teen Support Throughout Wyoming, We Have Continued to Expand – We Now Provide Support to Laramie County and Natrona County Teens and Are in the Process of Opening Up a Campus in Albany County in 2023-2024.USI Is a Prominent Figure in the Cheyenne Community and Burgeoning Organization in Casper and Laramie. in Cheyenne, We Are a Leader of the Laramie County Homeless Collaboration Where We Work With Other Agencies to Identify Barriers to Housing for the Homeless and Find Ways to Help Them Overcome the Barriers. Furthermore, We Collaborate With a Several Organizations Focused on Housing on a United Way Grant in Laramie County.
