Community Project Funding/congressionally Directed Spending - Construction - This Project Is Being Developed to Expand and Remodel the Current Infrastructure of the Health Clinic. This Will Allow for Sufficient Space to Provide Needed Services to Tribal Members and Allow the Opportunity to Develop and Better Serve the Tribal Members' Needs. the Sokaogon Chippewa Community Is Located in Rural North-central Wisconsin and Is the Primary Care Provider for Tribal Members and Employees on the Mole Lake Reservation. the Clinic Experienced Rapid Growth in Demand During the Pandemic Boosting Its Workforce Numbers and Putting a Strain on Its Space Capacity and Ability to Expand Needed Services. With This Project Expansion Efforts Will Enable the Tribe to Increase Disease Management With Additional Providers and Services, Increase Mental and Behavioral Health Service and Capacity, and Expand Alcohol and Other Drug Abuse (AODA) Services. in Addition, Sokaogon Entered Into a Memorandum of Understanding With Previa in December of 2023 to Improve Access to Care for the Community and Explore Potential Arrangements for Telehealth Services for the Health Clinic's Patients Through Prevea's Providers.
