Clear Creek County Is Seeking Funding for a Newly Constructed One-stop “collaborative Care Center”. Completed in July of 2021, the 12,000 SQ. FT. Facility, Named the Clear Creek County Health & Wellness Center Houses Primary Care, State and Federal Eligibility Programs, Mental and Behavioral Health Providers Accepting Medicaid, Clear Creek County Public Health Providing Immunizations for Children and Adults, Wic, and Other Cdphe Core Services. Combining These Services in One Location Makes Each One More Effective and Sustainable. We Are on the Road Toward Increasing Primary Care Visits From Our Current Standing Well Below the State Average, Behavioral Health Referral Follow-up, and Our Immunization Rates Through This Single Project. This Project Will Enhance the Health and Well-being of Clear Creek County Residents and Our Neighbors by Enabling Us to Finally Address Health Equity Issues That Have Been Present for Years. It Is Projected to Generate Excess Revenues Which Can Further Support the Health of the Community. This All-in Approach to Collaborative Care Could Become the Model for Rural Health Care Delivery, Which Is Why Many of Our Funders Around the State See This Project as an Investment in Rural Health Everywhere.
