Making Telephone Reassurance Calls And/or Visiting the Disable and Shut-ins. the Primary Focus Area of This Project Is Healthy Futures. at the End of the Three-year Grant, the Outcome Will Be the Number of Individuals That Reported Increased Food Security and the Number of Individuals With Increased Social Support. the CNCS Federal Investment $40,000.00 Will Be Supplemented by $10481.00 Non-federal Investment, and $4700.00 In-kind Monies.
