Support for payment for advanced primary care management (APCM) services in Medicare CY 2025 proposed physician payment rule. Support for shared decision making and care planning and services, and primary care quality measures, for Medicare beneficiaries with cognition concerns and their doctors, omitted in the CHANGE Act of 2023 (HR 4752/S 2379)/American Medical Innovation and Investment Act of 2024, HR 8816.
